Registered Blind?

Yes No

If you tell us you are registered blind then we simply need your name, address and contact phone number.  We can easily confirm your blind person's registration details with this information.

If you are not registered blind then you need to provide us with suitable evidence of your visual impairment.  This is not difficult, simply return the completed form which we will send you with the first edition of Hamilton Sound.  A doctor, optician or Social Worker can endorse the form and you may find that by contacting the local Social Worker for the Blind other aids and services can be made available to you.
